My mother was admitted to Lely Palms last week for rehab and skilled nursing. The rooms and halls are super clean. I was very impressed with the friendliness and helpfulness of the staff. There were no attitudes like those I experienced at a similar type of healthcare facility last year. Parking is easy and visitations are allowed anytime. Praying that mom continues to do well there and grows stronger.

I have never seen or heard that a facility can not explain anything to family memebers on a patient health status. Let me explain, Friday that brought her in. They didn't feed her, we bought her food. We left her their with no swollen leg and no bruises. We come on Sunday and we find her with these things. Not one staff, can give us details. They are acting like no we didn't know. But they showered her and didn't see her leg swollen with lots of bruises. We called the EMS and no staff could give them any paperwork. So we transferred her to a hospital. Please be careful and watch them daily. This place was to rehabilitate her not make her worse.

Horrible experience. Living conditions beyond sub standard. Facilities need a total overhaul. My mom was sent there following a hip replacement. Due to the conditions and care I removed her after a week. It was more detrimental to her health than helpful. I would never allow her back there and would highly suggest that anyone considering look elsewhere. My mother was released without any of her medications. Went back to get them and given no paperwork stating what they had been giving her … Found out she was not receiving her Alzheimer’s medication or the antibiotics for her UTI and is now back in the hospital be treated.

DO NOT EVER USE this rehab . They are unprofessional and unorganized their patients are on their own . My grandmother was transferred their Friday afternoon and we didn’t go visit her Saturday because we were cleaning her house and we went Sunday morning to visit and bring a change of clothes awe noticed immediately her right leg was all swollen and bruise and scraped skin on her left leg and not one of the nurses or charge nurse could give us a straight answer nothing but maybe or we don’t know whether or not she had fallen or if something happened and the nurse that was supposedly with her all night did not know anything about what happened. They didn’t call us or inform us about her leg we had to find out ourselves when we went to go see her . Her hair was in a French braid when she arrived when we went to go see her Sunday morning it was in a tangled and knotted lazy bun they showered her only because we had to call and complain to the head manager since according to them they have a “schedule in which they bathe their patients. The receptionist dose not know anything . I asked Friday afternoon when they would bring her food since she hadn’t eaten since she was transfered from the hospital and she didn’t know when they feed them or anything r ask one of the two nurses that were there in her room once she first arrived and one nurse had supposedly went to the kitchen to go see and she never returned we ended up having to go to Publix to get her food since they still had not fed her by the time we left that day and Saturday we have no idea if she was fed or taken care of properly we only no they took her out of bed to shower her . My grandmother was transferred their for physical therapy for her legs and hips and mobility issues and needs help getting out of bed and moving around and they left her their like a wounded animal .They do not care about their patients they care about the money they are negligent and shady covering up what really happened . We told them we were taking her out of there and they called the ambulance and even the ems said this was a horrible place the ems asked for the paperwork and nobody of them could give her or knew who could her paperwork they ems ended up taking her without any paperwork from them.

The facilities are dated and it is very depressing looking. The carpet needs to be replaced, the pool needs new lounge chairs, the game room is literally falling apart, etc. Everyone used to talk about the food being good and I agreed that it was but even that has gone DOWNHILL over the past 6 months. I know my grandmother is happy with her apartment and the renovations done on the apartment are really nice. I was hoping that HQ would be updating the common spaces but clearly that is not the plan. We are now researching other places to move her because the cons are beginning to outweigh the pros. Some of the employees that have been there for awhile are really nice, but HQ needs to step-up their commitment to this place, this is not their fault.

i was just discharged from this rehab on 2 days ago . I was there for over a month. It was horrible and would never recommend. The only shining light there is the Therapy Team. Janet and Justin are the absolute best. And on my discharge I was given prescriptions and needed medical equipment for my home. My oxygen is still not here. All my prescriptions were wrote wrong and after 2 days my insulin was finally received it was not called in with needles. No one answered when called. And when finally getting to Eileen she was rude and said to me what do I expect her to do. I had transportation home which is covered by insurance. they arranged transportation due to being in wheelchair, once locked in i s told he needed 100 dollars to be dropped off.Most of nursing aides are horrible taking a hour to answer call lights. one aide got the b d controller stuck under bed leaving me trapped on bed with bed off floor and feet in air and head down at 5am and said i would have to wait for maintenance to con at 8 am! I had to call my husband to drive 30 minutes to help me to he janitor helped me. and this isn’t half of it, including medication issue's with wrong pill given.

I would never recommend anybody's to put their love one in this facility. They're so negligent they treat the patient very bad they leave them with with the poop and the urine. The nurses don't have any compassion for them. I left my mom one night and the next day I took her out of there immediately. The only thing that was good was the gentleman from therapy. He's amazing and he deserves to be in a better working place because that place is not worth having somebody so good. The sheets had urine from the from the client before the floor was dirty with food from the person before, and the call button was not working this place was super disgusting dirty everything broken everything old windows, broken, moldy stuff and the food was awful no towels no soap. No nothing. I don't know how this place is still working or has any service or any recuperation supposed to help anybody when it's so bad.
