I would have given this place one star, save for some of the great people who work there (Chris, Jesus, Linda, Marilee, Yolanda, Aldo, Emily, Raynesha, Ashley, Rit.) It was disorganized from Day One - there was pretty much no intake process, and we had to ask a bunch of questions to get basic information (meal times, what type of clothes to bring, etc. etc.) The administrator was there intermittently, and the staff never knew when she would be in, or the social worker.

Whoever said it smelled was correct - an unidentified mix of bodily fluids as soon as you walk in. We had to ask multiple times for my father to get his medicine on his regular schedule. We had to ask multiple times for a CNA to change him or move him to the wheelchair so he wouldn't be in bed all day. No one seemed to be the source of truth of who was working and where they were at any given time.

Showers are only twice a week, and they skipped him AND tried to blame it on him refusing - not realizing I had been there the whole time and knew they hadn't come to get him. So they wanted him to wait several more days before getting one because they "preferred to stay on the schedule" (regardless of him not being clean.)

He had a rattling, productive cough and they ordered an X-ray, which they said showed nothing. Instead of pushing the issue, they gave him cough syrup. Everyone knows cough syrup does nothing but mask the underlying condition.

His roommate was suffering from a broken tooth, and had to beg to see a dentist and ask multiple times for his meals to be changed to something he could eat.

If we hadn't been there to advocate for him, I don't know what would have happened. It's a shame because there are some great people who work there and it's conveniently located, but they only got their act together when someone called the ombudsman and they knew the Health Department was coming for an inspection.

I recently visited this facility, and while I initially hesitated to write a review, I feel it’s important to share my experience. The facility had a strong odor of urine and felt unclean. The staff appeared inattentive, with many focused on their phones instead of the residents, which I believe is unacceptable. Based on what I observed, I would not recommend bringing a loved one to this facility.

I am very pleased with the help I received in this place. The nursing staff was awesome, the physical therapist was great, the recreation staff was amazing, I love the care and professionalism they have. I want to thank : Geni, Rosita, Alba, Dora, Tanya, Ritz, Fred, TJ, Denim,Kathy, Johnny, Xitlali, Tatiana, Francis, Frank, Maria, Maribel, Gabi, Jostling, Margarita, Jasmine, Ivette, Rosemary, Felicia, Dasy, Sidney Ramon, Aldo, Rouel, and if I forgot one of you, please forgive me, always know you all have a place in my heart. If I could give this place zero stars I would! Terrible place do not send your loved ones here night shift nurses suck and don’t tend to patients they have no tech communication between nurses and patient just a button with a light so nurses come when they feel like it . My mother was sent here and has had nothing but a bad experience from calling me distraught at 1am to tell me her medication is late and hasn’t been given to her which is crucial to her recovery after being hospitalized,not checking her blood sugar to not being changed after calling multiple times for 2 and a half hours going on three not being checked at all! I feel bad for the patients who can’t speak out if you have family take them out of there!totally unacceptable no human being should go thru that treatment…

I live right next-door to the facility, and let me tell you the amount of times that you hear patients screaming at the top of their lungs for help is way too many to count. Patient scream for hours on end for help. Cops have been called on this facility multiple times in the early hours of the morning and late hours of the night because no one answers their screaming patients.

Never again! If I could give a zero I would…Very negligent facility and an absolute nightmare for your loved ones. They neglected to give care, like toileting, PT and wound care, basic needs not met until insisted upon, forgot his meals several times, they had faulty equipment… gave him a broken bed with headboard on backwards and no call light to ask for assistance after asking everyday to install, my father can’t walk, he needs assistance to move, missing items like a tv remote,
admissions nurse was rude and literally told me she was just there for paperwork she would not address his medical needs, she took over 5 hours to admit him and didn’t order his medications in a timely manner, resulting in my having to go my fathers home to get his meds or he would not have had them that night,
Later they overmedicated my father, with Alzheimer’s, on an antipsychotic medication, I can’t tell you how bad that is, it could have been catastrophic, he was over medicated the entire time there!
no return calls from facility physician on orders or an in person visit for 3 days, no follow up from charge nurses as issues came up,
he had a fall in his room that no one witnessed, he still had no call button…
and nursing supervisor didn’t want to call for help until hours later upon my insistence. They did not want to call an ambulance emergency. Only a non Emergent ER ambulance transport. They wanted me to take their word that he was ok. How they could assess that without seeing the fall or any X-rays, tests exams
is impossible. Went to the ER by ambulance after another charge nurse stepped in 3 hours later and called 911. We found out at the ER he was hurt during the fall. So much more happened. I can go on for days! This all happened in a period of 4 days. We had to urgently have him discharged after day one and they delayed the process for 3 days! Where all of the continued neglect mentioned above happened. Only upon a threat of calling Medicare and department of health services did anyone finally give me the orders needed for him to get released.
Then I got the order In 30 minutes. But the order was too late he had the fall the morning he was to be transferred out if there. Convenient right? After speaking to Administrator of facility all she could say was she had no idea this was happening. Even after days of reports to charge nurses and social worker at facility in person and by phone. They should be shut down. Never have I seen such negligence, I’ve been in healthcare for over 20 years..and I still had to move mountains for care. My fathers recovery is going g to be delayed and we’ve suffered a lot of out of pocket expenses for all the tests, ambulances, equipment needed to go home, private caregivers as result of their negligence.

My SistaFriend & I arrived a little early for our allotted appointment, the staff checked us in, asked questions as required by their job. Very professional and nice. We were escorted to our love one room and he was surprised to see us. Staff help keep the secret friends from LA and Texas was coming to visit. Best time of my 3 day vacation while visiting hometown of Los Angeles. The staff also gave us required face masks due to Corona Virus to put on and the site was clean and I didn't smell anything out of place. Take care, Brenda from Texas. 🌹🌹🌹🌹🌹

If this facility had lower than 1 star, that’s what I would give them. My brother in law was in this facility and was not treated very well. There’s not enough nurses available to help out with patients. When I asked for help, they would say, ok, we’ll be right with you, and we would have to wait for 1/2 an hour. His wife bought him a new electric shaver, which he never got to use. The facility said it was misplaced- and never found it. This is a horrible place!
