I agree with all those who have said this is a terrible place. Two staffers "dropped" me in the bed the second night and I begged for an x-ray. After 4 days, got one and had a sprained knee. The food is inedible (too much salt) and looked terrible. I was to get rehab after vascular surgery. Waited 4 days. Left AMA. The staff walks by the door and never stop; they ignore the "call button" (if that what it is). There are different people every day and they woke me up at 4:30 or 5:00 in the morning for wound care (which I only got twice, and they did not even look at the groin stent). Don't send your family member there!

Elder abuse. Broken toilet seat when the patient is using a walker and already in danger of falling. Reported 4 times and never repaired. The food- you wouldn't give to the dogs. My grandfather rang his bell and didn't get a response from a nurse for 3 hours- they neglected to give him the medication he needed! It's incredible how they could get away with this. My grandparents are very shell-shocked from this abuse.

Horrible place. Advent Hospital gave a list of rehabs and this was the closest to home so we selected it. Cannot begin to tell you how horrible. My friend arrived there on a Saturday night 3 days post surgery to repair broken leg. As standard protocol the hospital will not let you leave post surgery without using the restroom.My friend did so with out any issues and was then released to Ormond Rehab. Their standard protocol is to give each new arrival a stool softner. This unneccessary medication caused her to have nausea and uncontrolable diarrhea sending her to the hospital 8 hours later. The nurse at the ER determined she had been given stool softeners for no medical reason other than "protocol" 2 weeks now and her bed linens have not been changed. I have now called 3 times and left messages for the administrator Donna Smith, 6 times left messages for the "case manager" 4 times the staff on site wrote "notes" to leave on Donna's desk and or the case manager. Not 1 person has returned a call. Night staff says they do not have access to patient records and cannot tell us what medications they keep bringing to her in an unmarked cup. They also cannot tell us on the weekends when her appointment is scheduled to meet with the orthopedic who did the surgery. She has only been taken to therapy 3 times in 2 weeks and each session was 10-15 minutes. No structure and ZERO care from the staff. The smell of urine and fecese hits you in the face when you walk in. Elderly patients missing limbs are sleeping or roaming the halls incoherent and under no supervision. The call buttons go unanswered for HOURS! my friend called me from the toilet after 27 minutes of waiting for someone to come help her. I have called the Department of Elder affairs for an investigation but BE WARNED not to let loved ones be sent there!!

i advise people not to take or leave there loved ones there. Thry are not cared for left with dirty diapers oor naked on a cot on the floor . and they are givven so much medication they are doped up all day. If you love your parents grandparents that is not the place Nurses care do not care .please take it from experience go else where.

My father was there. They left him drugged and naked on a bed. If you want your love ones to die, send them to this trashy place. No rehab nothing, just left on a bed. Disgusting.

We had transferred my father in law from a hospital after numerous falls to Signature Healtcare of Ormond for physical rehabilitation. The staff treated him like family and the physical therapy was top notch. Surprizingly the meals prepared by their chef were restaurant quality making his stay so much more enjoyable. Thank you to all the staff for his care! I would highly recommend Signature Healthcare of Ormond for physical therapy.

BEWARE! The hospital provided me with a list of rehab centers for my father. This place had 5 stars and US News & World Report listed it as one of the best. Not being from the area, I relied on that information. CAUTION! Those reviews are from 2022 and previous ownership (Signature). I fought to keep my father from going to this place. The front doors don't close properly, lady at the front desk was disturbed that I was there, they were hesitant to give me a tour, elderly slumped over in wheelchairs throughout the halls, walls in the rooms were damaged. There is not one picture on their website that properly shows their current state. People, take time to visit BEFORE making a decision.

I honestly do not know how this place could get so many stars. I visited the Rehab facility today to admit my wife after she fractured her neck. The place was a bit tattered on the entrance exterior. There is a flag pole outside with an old faded American flag and I believe an MIA flag. A bit disrespectful in my opinion. The lobby was also a bit tattered with old and scuffed up furniture, as well as the floor. I noticed damage to the walls with paint pealed off in numerous places. I know I know, that all sounds petty, but wait there's more. As I waited for the receptionist, an angry women came up to the reception desk complaining about the nurses ignoring her when she inquired about something. The admissions woman gave me tour of the facility. The so called dinning room, which is also the activity room, is small. Walking down the hall passed the patient rooms, the place wreaked of urine. The rehab room was also small and the only equipment they appeared to have was balls and some small weights. Not what you would expect from a place that should have physical therapy equipment. Throughout the place I kept noticing damage to the walls, including pealing paint and lots of black scuff marks on doors from wheelchairs and such. Walking past the patient rooms again almost made me sick with the smell of urine. The cramped facility was load with commotion and havoc. This was the short term Rehab area. I can't imagine what the long term nursing facility is like. I was only there for a total of about 20- 30 minutes, but it was enough for me eliminate this facility from my list of possible rehab places for my wife. I have no idea what the staff and care is like here, but the physical conditions are enough to prevent staying long enough to find out.
