At the beginning of my visits I was told I would be seen by a nurse, PT and OT.
I saw 1 nurse, Melissa for the first visit. Later, I talked to 1 nurse on the phone and answered 2 questions. Then texted with 1 nurse about an appointment. Never heard from or saw another nurse again.
Did not want OT so my PT guy informed the office and was able to check and change those visits to all PT. Which was exactly what I wanted.
Both my PT guys, Jake and Jeff were great. Were very professional and patient with me and all my concerns.
If it wasn't for the nurse issue, you would get 5 stars.

April is such a professional Therapist while making sure her patients know she really cares for them and wants them to truly recover and heal correctly.

The team of professionals at Center well of Orange Park have become a lifeline in the past two months!! Jennifer alerted me to a dangerous issue I was unaware of and got me to the ER on time. Davida was also a valuable nurse that became like family to me with her caring visits. Sheree another nurse I would like to give much thanks. She went above and beyond her duty in caring for my mental health needs as well as physical needs during hurricane Helene. And finally, Kevin got me moving around in no time with my P.T
