I am deeply disappointed with the state of Terrence Cardinal Cooke Health Care Center located at 1249 5th Avenue in New York City. Our experience at this facility is far below expectations, and I feel compelled to share my concerns.

First and foremost, the condition of the facility is appalling. There is a serious rodent issue that cannot be ignored. During my brother's stay, there have been sightings of multiple rodents scurrying around in different areas of the building. This not only poses a health concern but also creates an environment that feels unsafe and unsanitary for residents and visitors alike.

To make matters worse, the smell inside the building is horrendous. It’s difficult to describe the overwhelming stench that lingers in the hallways and common areas (6th Floor) —it’s unbearable. It’s clear that proper sanitation and hygiene are not being prioritized, which is incredibly concerning for a healthcare facility.

Overall, I would strongly advise against using this facility until they address these serious issues. The lack of cleanliness, combined with the rodent infestation and the terrible smell, is unacceptable for any healthcare center, let alone one that’s supposed to provide care and comfort to vulnerable individuals.

Nurses are great. Had to get meds straightened out. Rehab was fine. On a scale of 1-1] the nurses a d CNAs were a 6 combined. When I came in I was not walking

•There were clearly a Few competent individualsw in each department and at various levels of service, but Few. Stopped them from giving out WRONG meds on more than one occasion, an unsupervised trainee here, a regular staff RN there.
• We found no cohesiveness, just individuals getting their job done, sometimes at minimum level, sometimes better.
This is apparent from the top levels downward, in fact we saw some of the most competent and caring attitudes exhibited at the lowest pay levels, treating patients with proper care and respect.
•We left after a couple weeks, Much Worse for having been there, and thankfully found a truly caring, competent, healing environment in the next subacute rehab.

I was here in 2008 when I broke my ankle and when given the option to come back I said Yes. I have no complaints from anyone. They had Sundae Sundays with ice cream chips. It was an amazing experience. Thank God I was able to return.

My father was sent here after receiving open heart surgery at Columbia Presbyterian. To be completely transparent, this place is in terrible condition. There are literally mice running around the halls and in patient’s rooms. Virtually everything is filthy to the touch. Being located near Central Park in a rather affluent area does not change that. Some patients were smoking in the lobby before I went upstairs to see my father. Most of the staff (not all) are neglectful of the patients, most are unprofessional, and the establishment is not fitting for any rehabilitation whatsoever, as there is no real rehabilitation being offered to anyone. The patients are often more friendly and approachable than the staff. My father has yet to receive any physical therapy, and when I went to visit him on his first day there, no staff had addressed his soiled gown and sheets. He was in an uncomfortable posture (I have no idea how long he was in that posture considering the dry vomit on his gown) and did not have the bare necessities provided to him at Columbia Presbyterian. The food is atrocious. My father’s condition is seemingly worse than when he first went in, and I am very concerned about that. The water running from the sink in the room was very concerning as well, but not surprising when you consider the overall condition of the place… I will do everything I can to get him removed from this Hell hole. If you want your loved ones to survive, please do not allow them to stay at this place. I highly doubt that they will survive the duration of their stay there. Archcare only receives one star, because there is not an option for 0.

Update: Three weeks later and my father is back in the hospital… What is seemingly nursing home negligence, has led my father back into the step-down unit… I sincerely regret ever letting my father set foot in that place… DO NOT LET ANYONE YOU LOVE STAY AT TERENCE CARDINAL COOKE. They can and will die there…

Update: My father passed away on July 6th 2023... He passed away roughly a week after his stay at Terence Cardinal Cooke… Again, if you care about your loved ones, do everything in your power to keep them out of that Hell hole… That place needs to be shut down…

After much thought, & time working there, i’m giving this place 2 stars. i won’t lie, i enjoyed my time working there, but not for the reasons one would think.

When i first came in for orientation, i was greeted by security who was honestly very friendly towards me. Orientation was pretty good also. i ended up being placed on 7, won’t say which side. i was trained by a really nice lady who was very tough and helped me gain confidence.
Now with that being said, within the course of 3 days, i noticed that a lot of residents complaints were being swept to the side. now i know sometimes residents are a bit overbearing, but this is our job to take care of them. often times i was told by older aides to just ignore the noise and would hear them suck their teeth. it was so sad because i heard two residents make statements about how they just wanted to die, but when i went to speak to them privately, all they wanted was to be listened to. i think this is important to maintain a nurturing and kind aura when you speak to any resident, because they are vulnerable and in need of the utmost care. anyways, i was mostly on one floor, i did get kicked once while on the hospital side, but that’s to be expected in my opinion, i survivedšŸ¤·šŸ½ā€ā™€ļø.

depending on what side of the facility you are on, it’s actually very clean, renovated (the hospital side, dialysis, etc)..some areas are nasty. 7 was alright, there was a smell but i got used to it, as stated that’s to be expected.

there were a lot of older aides who i noticed were very bitter over the younger aides, would often blame them for not answering call bells and changing diapers when in actuality it was the older aides. most times, i’d walk out of a residents room to literally 4 call lights on, and the aides sitting at the nursing station, and as soon as i went up and asked ā€œhey, what’s up with the person in that room?ā€ i was told, and i quote: ā€œoh can you go check it for me?ā€ā€¦NO i cannot, i have residents of my own to look after, and most times id end up checking anyways out of the kindness of my heart, a couple times i saw people on the floor and they just simply needed help!
a couple of the residents were a bit overwhelming, but i always chalked it up to the fact that they need us and that’s totally ok!
To wrap up this review…
i don’t have a single complaint about the residents, every single one i met (even the one that kicked me, LMAO) was a delight to work with, and they loved me because i treated them as human beings instead of another body. the AIDES on the other hand (not all of them) are a piece of work, they will indeed get threatened by you if you don’t do things their way, they answer the phones with attitude, & 60% of the time, they bring their bad mood into work. I didn’t interact with management after i got hired, not that i needed to, so can’t say anything about them!
Overall, i enjoy working here because of the residents, personally, i wouldn’t send any of my relatives here. Thanks for reading.

Dirty rooms with broken furniture, no communication for Admin and rude staff. Billing will find you quickly, but not social work, physical therapy.
