The care here is amazing! I was admitted to Avamere Crestview for about two weeks. The entire time I was cared for by a team of amazingly caring and compassionate nurses, CNAs and Med Aids. Their physical therapy team was amazing in helping me get to a level of physical health to go home safely, and made sure I was set up for home healthcare to continue my progress when I left.

The administrative staff were very kind and helpful in navigating my first stay in a skilled nursing facility. They made sure I felt comfortable and that my family did as well. They also made sure I understood their processes during my stay and made sure I had a good understanding of their discharge process for going home.

It is very clear that every staff member cares about the work they do, are diligent about ensuring that their patients have all they need, and make sure their patients are as comfortable as possible. They take the time to get to know their patients and build the trust and confidence needed to provide a healing environment.

Like many skilled nursing facilities, there is a staffing shortage, but it's clear that when choosing staff, management chooses well. From the cleaning staff, care team members, and administrators, everyone is there to help in any way they can, and as quickly as they could. When waits were a little long for my call light staff were apologetic and did there best to address all my needs as fast as possible.

The building is a bit outdated, but while I was there maintenance stopped by more than once to see if there was anything they needed to address. The rooms are well maintained and kept to a higher level of cleanliness than I experienced at the hospital.

If you or a family member is sent here, you can rest assured they will be cared for and treated well.

The only critique I have would be the food. While the menu would sound good, execution of the dishes were not the best. Breakfast was the best meal of the day when it came to hot meals. Most days I had my husband get me lunch and dinner from restaurants nearby, or had family cook and bring me a meal. The a la carte sandwich options were good and were a good alternative to the cooked meals as well. The nutrition staff did do a survey with me before I left and I was honest about my experience. They were receptive to my responses and I feel they genuinely wanted to improve the quality of food at the facility.

Staff was very good and caring - so important in this setting. I appreciated the care and support from everyone, from the CNA’s to the nurses and especially the physical and occupational therapists. They got me started on the road to recovery, and I’m so thankful for what they’ve done.

Non responive staff nember at night. I try get their attention, with the cal light, only to be ignored or told to go back to sleep. They don't urn off the room light either. I'm housed there I cannot move around on my own.

I am a caregiver for a client who spent nearly a month at The Avamere, and they were wonderful with her. They offered her many accommodations to ensure her comfort and ease. I felt like she was in great hands, and when I stopped by to visit, it was definitely confirmed. I recommend the Avamere, especially for anyone with Intellectual or Developmental disabilities. The staff are extremely kind, patient, and willing to go above and beyond for the comfort of their patients.

My mother was in rehab for almost a month. We found the staff & the Aministrator, Marcus, to be very caring & efficient. My mother's care was excellent. Laurie in admitting was very helpful & Jose not only cleaned the room but cheered us both up. The food was good & help in the dining room was excellent. This was my mother's second visit & we will return if necessary

I visited a family member here and found it such a welcoming and helpful place. Very friendly and capable folks. I also liked that they had coffee ☕️ and water for visitors. Plus some free snacks too!

Let me start by saying it would be really really hard for any nursing home to get 5 stars...It is just the nature of their business. I do feel that this facility does a lot for the resources they have. We have been very pleased with the staff, especially Mikey, and the CNA team that cares for my mother Mrs Joyce Wilson. I can recommend this facility.

I should have written this review years ago, but my father died at this facility in 2009. The day of his death, his lungs started filling with fluid and I asked the staff if they could remove the fluid and make him more comfortable as he was having difficulty breathing. It took a while for someone to find equipment to do this and when they finally did, no one knew exactly how to use it. As soon as he passed away later, an Asian women looked at my mother and asked her what she wants them to do with the body. I should have hit her. I would never recommend this facility

Trying to find skilled nursing for yourself or loved one is a very difficult thing to do. I was very hesitant to send my mother somewhere but after a fractured hip and surgery, I felt I was not immediately able to care for her at home. I was told Avamere is a decent facility which I why I agreed to send her there. Well in less than 5 days she had fallen again and broke her wrist in two places. The nursing team said repeatedly that she had full range of motion and they had evaluated her and that she was fine. I am an RN too, it was very clear that she did not have full range of motion and very apparent that something was likely broken (swollen and bruised). I had to push them to get further evaluation with an X-ray which happened two days after the fall. There were no fall precautions placed on my mother even though she was in there for a fall that resulted in a hip fracture with advanced dementia. Anyone who read her chart once would clearly understand she was a fall risk. Everyone acted very nonchalant about the fact that she fell and broke her wrist. They also acted as if they had no clue she was at risk for this. Truly despicable. I’m not even sure anyone really offered an apology. This place is the tired cliche of places that take people, charge so much money, and even provide basic safety. You hope that at minimum your loved one is safe - never mind actually receiving the services that they say they will like PT/OT, etc.

I would never send my parent back here nor ever recommend anyone send someone they love here either, especially if that person is vulnerable and cannot advocate for themselves. All of this happened despite my visiting every day for hours and understanding the horror show that is the elder care system.
