My mom has been in this facility for 3 years. She is very happy here and the staff truly loves her. They try their best and when we encounter an issue they take care of it. The staff always responds to emails and concerns we may have.

St. Augustine Health & Rehabilitation Center is a commendable facility. I stayed here following an accident this summer after surrounding facilities would not admit me. While no facility is perfect, I was very pleased with the care that I received here. From administration and admissions to skilled nursing (none of whom are agency) and therapy, the staff is consistently kind, caring, and attentive to resident needs as best as possible. Under Tom’s administration, I will recommend this facility to anyone.

After reading the reviews I was very apprehensive about placing my dad here, but it was the only place that would take him the day he was released from the hospital. We even considered moving him, but I’m so glad we didn’t. It’s not the newest or the fanciest place. But the facility was clean and everyone was very considerate and caring for my dad, even when he wasn’t the most cooperative person. He was always clean and usually out of bed, he was in therapy or sitting in a chair. Thank you to the staff for being so kind and caring to my dad and to the rest of the family as well!

I don't know where all the negative comments come from but I have not had any problems with this facility with my love one and if I did you all would know it. When people help other people someone is always disappointed but that doesn't mean the entire facility is bad. Get up and go visit your love ones and understand they are in a nursing home facility and not your home. Question is why did you put them here and leave them. Apparently you trusted the process now continue to watch the process unfold before your eyes or not. This is not your home but a nursing facility where everyone that comes here for care deserves some attention

Wow. Is this the Worst Facility in Florida? The staff there will not help you speak to your family member they're they are always too busy. Now the place is refusing for me to speak to my mother I guess it's a rule you're not allowed to speak to your family members there on the phone. This place is more like a silent prison. If you value your family member you would not bring them here! My mom is stuck there and I was trying to save her dog's life that I am taking care of and they refused to let me speak with her or find out any information on her Dogs health . That is the most nastiest spitefulness thing I've ever heard in my life this place is utterly ridiculous! An i want to it's been a year later or more they haven't let me speak with my own mother. An there's a chance I might pass away An I can't even talk to my own mom! This place should be shut Down!

Do not take your loved ones here! This place is a death trap. Your family will be left in bed for hours on end and be dehydrated because they can’t reach the water and or the staff do not encourage it. The care at this facility is horrid. It’s infested with rats and mice, of course roaches and snakes get in from time to time. I reported that I seen a rat in bed with a resident literally eating at her brief which was soiled and the resident was bed ridden meaning could not move without assistance and they disregarded the information resident died a month or so later.I’ve seen bed sores emerge almost to the bone of a resident and they don’t have sufficient supplies or wound care to treat the residents .This facility doesn’t care ,isn’t safe and very unprofessional. Do not take your loved ones to this facility nothing has changed and nothing is better. They will steal from your loved ones and abuse them mentally, and physically .

This place should be shut down. I was lead to believe they were going to help us and they lied. I would never recommend this place or Jennifer in particular. We were unable to be with our loved one close to home due to them and were forced to be miles away.
They were not up front and lead us on. They cannot be trusted! There are alot of places that would like you to believe they've changed "recently " how convenient! If only they would be there when you need them. My loved one passed a hour away because they could not help us be closer. I pray no one else has to go through the process of deceit they played on us at a time we needed to be closer. They knew she was in need of help and didnt want her with them and that we wanted to be nearer at a crucial time. We ended up in an ungodly place and I cried the whole hour home alone when I realized she was leaving me.
