There are some people in the world who go to work, do their jobs and go home. Others take those jobs to a whole new level entirely. We, as a family, could not be more thankful that our mother was in your loving hands are CREEKSIDE POST ACUTE. We thank you for the care that was so beautifully provided. When you carefully wiped our mother's face or spoke words of comfort, you may not have realized it, but your actions not only touched her, but in turn all of us. Words can hardly express what that means to all of us. Thank you Marie, and your wonderful team, for the excellent, compassionate care you all provided for our mother. I believe you all have a very special angel looking over you now. God bless.

Because of the negligence of the nurse's staff is the reason why my grandma passed away.... They didn't bother to do better.Basic assessment on her, she was severely impacted, and by the time she went to the hospital she was on life support in the icu.... if the nurses actually did an abdominal assessment and saw that she was distended, they should have been able to send her to the hospital days ago.... Then they had the audacity to lie on their documentations and say that everything was fine with her, but yet at the hospital she was septic... her urine was the color of prune juice... my grandma would often call me and tell me that her call light was on for hours and she was sitting in her feces for hours... i would have to call the office and ask them to check her... this place is filthy, and the staff is horrible

This place is 0 stars but that option is not available. This place is FILTHY, CNAs not doing there job, hiding call lights from patients so they don't have to be bothered. One noc shift Asian RN believe her name is "arley" but not positive, she is HORRIBLE. PICTURES ARE AVAILABLE of her sleeping along with cna's. Your family will not get the care they need here find another facility. I can't seem to post the pictures but are available. Sincerely ex staff

Thank you Creekside post acute for taking good care of my Dad. Excellent services from the reception Ronald who is very friendly and helpful. To the CNA’s: Trish, Melody, Amber, January, Louise, Stephanie, Tory and Cristina, Lena, Sandra and Jessica.

My grandfather was admitted to this care center, and we couldn't be more happier. This facility is under new owners and new managers and they are now called Creekside Post Acute. I am so glad we found this place. My grandfather is now back home with us and all he can talk about is how caring all the staff was especially Marie, the Administrator. The managers are all new but they sure know what they are doing. Every time we visited the place was always clean and smells good. Just want to say "Thank You" to Team Creekside Post Acute for a job well done! My grandfather is not an easy person to please but you guys won his heart over. Would highly recommend this care center.

My uncle was a patient at the facility and I have to say I've never seen a better team. The cnas we're caring, the nurses answered all our questions and concerns. The rehab team to really helped my uncle be able to be walk without that much help. The social worker was very attentive and showed real compassion towards the patients and the families. I would definitely recommend this facility

My mom was placed as a resident at Creekside some time ago after coming out of a medically induced coma. The staff there was very attentive, compassionate and made her feel comfortable. When she first was admitted she had very unsteady gait with drop foot, with the help of the PT dept and the CNA she regain the strength she needed to walk properly without any assistive devices . With out the hard work from the staff and the encouragement they provided to my mother she wouldn't of been able to go home nor would she have made such a quick recovery.

I will make this short. Betty was transferred to Creekside January 27, 2023. She had been in ICU For 3 weeks and 1 week in a room on a ventilator. When Betty arrived at Creekside she was immediately evaluated by the Respiratory Director Rocky Chavez, Nursing Director and Physical Therapist. The professionalism was excellent. Betty’s recovery was indeed a miracle with God and the expertise of the staff of Creekside. A final note Betty and I live in Corona 40 miles on away. We could have moved closer but Creekside care and professionalism made it a no brainier.
